@MAbouljoud@shimulshah73 It seems there are two flow issues: stenosis from residual thrombus and diversion of flow through collaterals. Agree with heparin and VAC closure, but maybe IR can help with portal/SMV angioplasty and occlusion of collaterals. PV otherwise looks good.
