Cemented vs. Uncemented Hemiarthroplasty for Displaced Intracapsular Hip Fractures: An RCT

Research Question
In elderly patients with intracapsular hip fractures, how do cemented and uncemented hemiarthroplasty compare with respect to pain, mobility, and mortality?
Results
Mobility: the cemented group demonstrated significantly greater improvement in mobility during the first year post-operation. No differences were observed at 3 years follow-up.
Pain: the severity of pain was similar in both groups, up to 3 years follow-up.
Mortality: the rate of mortality at both 2 and 3 years was higher in the uncemented group, trending towards statistical significance (p=0.053, p=0.060, respectively).
Takeaway
Cemented hemiarthroplasty appears to provide greater improvement in mobility in the short term, and a lower mortality rate 3 years post-operation compared to uncemented hemiarthroplasty!
Patients suffering from a trochanteric hip fracture may benefit from a preoperative CT. Preoperative CT increases the orthopaedic surgeon’s ability to detect unstable trochanteric hip fractures.
