In some countries it's mandatory to pay even if unemployed or you get hit with fines

MOST jobs get you insurance by default.
Check Germany's healthcare. Mandatory payment regardless and still barely covers anything for "free"

Oh and if you have insurance your healthcare is a non-issue.
("Free" or "universal" healthcare is a monthly payment too)
