Clinical outcomes among HIV positive babies below 18 months of age, diagnosed under the Early Infant Diagnosis (EID) programme, India: a mixed-methods study
Summary
Early initiation of antiretroviral therapy (ART) in infants with perinatally acquired HIV improves clinical outcomes and survival. In India, Early Infant Diagnosis (EID) was introduced in 2010, but its impact had not been previously studied.
Methods
A mixed-methods study analyzed data from 310 children diagnosed with HIV before 18 months of age across 30 ART centers in 11 states. Quantitative data were collected retrospectively, and 67 in-depth interviews with healthcare providers and managers provided qualitative insights.
Findings
Treatment and Outcomes: 64.5% (200/310) of infants were on ART, 11.6% (36/310) were lost to follow-up, and 8.1% (25/310) had died.
Timing: Median age at diagnosis was 231.5 days, and median ART initiation age was 309 days.
Survival: Three-year survival was 91%.
Risk Factors for Mortality:
Low baseline CD4 count (<1500 cells/mm³): Hazard ratio (HR) 11.39 for mortality.
Advanced WHO clinical stage (III/IV): HR 2.42 for mortality.
Opportunistic infections: Higher risk with low CD4 count (HR 4.87) and advanced clinical stage (HR 1.92).
Barriers to ART:
Demand-side: Stigma, discrimination, and parental desperation.
Supply-side: Remote access to pediatric ART centers and lack of child-friendly ART formulations.
Interpretation
Children who remained on ART had good survival rates, but those with poor immune and clinical status had higher mortality. Ensuring retention in care is critical for better outcomes.
