Atrial flutter (AFL) with varying degrees of LBBB, most likely due to left bundle branch Wenckebach periodicity of the partially or completely concealed type (Rosenbaum, 1969). Because of the AFL, successive impulses periodically reset the cycle, resulting in two alternating LBBB morphologies. This phenomenon is considered concealed (hidden) or partially concealed because a progressive widening of the QRS complex is not always apparent on the surface ECG. Instead, two distinct LBBB patterns alternate in a consistent and repetitive manner.
Phase 4 block induced by a PVC. When a subsequent impulse reaches diseased conduction tissue during phase 4, conduction may fail because spontaneous depolarization makes the resting membrane potential less negative, reducing sodium channel availability and preventing normal depolarization. Similar mechanisms may also be triggered by a PAC, termination of tachycardia, slowing of the sinus rhythm, or other conditions.
