@tarunjindal3165 @JasmineNephro @NEJM Since creat is high, both the kidneys are involved and it's not a unilateral disease. Look for proteinuria/hematuria. Evaluate for other causes of CKD including autoimmune, complement levels. Try to add low dose ARB (monitoring electrolytes) along with CCB etc

@drabdulhameed07 Why would you want to control the BP rapidly. It's HTN urgency. You need to look for any end organ damage and if no then optimise the oral drugs as rapid decline is not recommended and no more than 20-25% in next 24 hours.
