Interestingly, even though the rate of pathologic complete response (pCR) was higher in the 12wk group, the rate of sphincter preservation was lower. The true benefit of a pCR is unclear to me - does this consistently translate into longer disease free survival?

Propensity controlled models do nothing to balance unmeasured characteristics, which may result in unmeasured confounding variables being present, thus biasing results. Prospective, randomized, blinded trials would be more likely to change my clinical practice for this issue.
