Last case before holidays.
🦀 Medical record: anal neoplasm.
🦵🏽 Subcutaneous lesion on the leg.
🖥️ Hypoechoic, irregular edges. 📏 36mm.
💉 Ultrasound guided FNA / CNB performed by Interventional Pathologist (Dr @LangerPath)
2️⃣5️⃣ and 1️⃣4️⃣ g.
🔬 Metastatic Squamous Cell Carcinoma.

💉 Another Pathologist is completing her training in Interventional Pathology within the Pathology Department at @HUCA_Asturias.
👏🏽 It has been a pleasure to share our experience (US FNA and US CNB) with Dr Tamara @LangerPath from Hospital Universitario San Pedro @riojasalud.

Thank you for sharing your experience and your teaching. You’re an amazing team!🫶
Whising to put both procedures into practice in my hospital!

CC391. Gloomy Sunday case. Organ and diagnosis? When I show you the histological image (the day after tomorrow), it might become clearer to you. In my opinion, few tumors—if any—look like this after FNA of a XYZ tumor (adult). In a few years AI will spot it in seconds #cytopath

One of our brilliant residents showed me this case. Supraclavicular lymph node. No other clinical information at the moment.

Micropapillary to papillary structures, some mitoses, some mucin production.

BMB 41 yo male. All these cells are positive for CD3, CD5, CD2, CD7. In the pics you can see the CD4 and CD8. What is your diagnosis? What other stains would you like to see?

More on anaplastic large cell lymphoma (ALCL) mimickers:

Enteropathy associated T-cell lymphoma (EATL) may exhibit anaplastic morphology, with pronounced inflammatory background consisting of histiocytes, eosinophils, small lymphocytes, and plasma cells. Furthermore, EATLs with large cell or anaplastic morphology are often CD30+ and sometimes EMA+.

In the setting of a localized mass to the gastrointestinal tract and history of celiac disease, EATL would be on top of the differential, even if the morphology and strong CD30-positivity are reminiscent of ALCL.

However, in rare instances, where EATL with anaplastic morphology presents with involvement of extra-gastrointestinal organs, this could cause serious diagnostic challenges, as illustrated in this report @Histo_Journal➡️https://t.co/Xd36tXNFBb
