Always trying to make sure I “own the book so I can throw it away” and be fully present with my patients.
Listening to Jonathan helps. Highly recommend this one!
An eye opening conversation & cutting critique of current trends in the practice of psychotherapy.
Eternally grateful for @JonathanShedler’s generosity.
This convo has been on my aspirational list since I started the pod in 2023.
Find it wherever you get podcasts!
I'm a bit annoyed that NYT has settled on the title “Here’s What Psychiatrists Mean When They Say You Have A.D.H.D.” (presumably because it got more engagement) rather than the prior title “We’re Thinking About Mental Health Diagnoses All Wrong,” because the piece isn't really about ADHD. It's just an example I used in one passage.
Best summary of what psychiatric diagnosis is/& what it is not.
Should be read (& often reread) by every clinician/patient/trainee/family member.

I hate the phrase "dopamine hit" - dopamine is about incentive salience and prediction error, not a literal "shot of pleasure" every time you see a notification or eat a cupcake.
Let’s stop turning complex neurobiology into a buzzword. DOPAMINE DOESNT WORK THAT WAY.
The people who behave in this manner do a disservice to everyone, including those who are actually autistic. A big contributor to the collective knee-jerk reaction of distrust and skepticism towards "autism" is due to these self-appointed diagnostic evangelists.
Self-interpersonal psychopathology finds cover and refuge in whatever diagnosis it can, whatever diagnosis will allow the person to avoid confronting the nature of their pain and the self-repeating patterns of their lives, whatever will make for a more appealing & dignified story
You are starting with the assumption that psychotherapy is about“therapist-guided practices” and “adherence.” That’s not my understanding of psychotherapy.
Tackling a 92-page #delirium guideline can be daunting: https://t.co/HHxykJhgkx
Well, now there's a 6-page clinically focused review!

Keith Connors was "Father of ADHD"- performed first stimulant trial 60 yrs ago & guided DSM definition.
Just before dying 4 yrs ago, he & I wrote his BMJ obit- Keith's last words from the grave warning against massive #ADHD overdiagnosis/overmedication.

Your brain on stimulants vs your brain on ADHD
There is more to what ADHD is than what stimulants do, and there is more to the clinical use of stimulants than the boundaries of ADHD

I'm very troubled how complacent most people are about the threat chatbots pose to our patients & our professions.
This interview explains why & ways we can respond:
What happens when artificial intelligence meets psychiatry?

Great News
UK govt will study explosive fad overdiagnosis of Autism & ADHD. This is what RFK Jr should be doing instead of anti-vaxxing.
Main drivers of ovrrdiagnosis used to be loose DSM critrria/Big Pharma/careless MDs. Now its internet self-diagnosis.
