@IM_Crit_ If PEEP at 5 and ABG is ok I will presume the Fio2 is below 45, if there is no concern for increased ICP and there is no any other way to keep the tube, I would continue to liberate the patient from the vent and wouldn’t worry about the driving pressure whatsoever

The diagnosis is degenerative disorder 2/2 genetic/prtein clearance dysfunction, metabolic or inflammatory. Less likely neoplastic or vascular
