When pressure around the heart becomes the emergency.

Fluid accumulates in the pericardial sac → intrapericardial pressure rises → the cardiac chambers cannot fill properly → stroke volume and cardiac output fall.

Classic presentation includes hypotension, raised JVP and muffled heart sounds (Beck's triad), along with tachycardia and pulsus paradoxus.
ECG may show low-voltage QRS or electrical alternans, while echocardiography is key for rapid diagnosis.

The principle to remember: it's not simply how much fluid is present-it's how quickly the pressure builds.

When hemodynamic compromise develops, cardiac tamponade is a life-threatening emergency requiring urgent drainage, usually pericardiocentesis. #neetpg #fmge #Inicet

Dr. Dennis Masoba Nyambane, a general surgeon working for the Kiambu County Government at Kiambu Level 5 Hospital, is in custody for allegedly stabbing his wife to death.

According to the Police report, Dr Masoba attacked Mary Mwende Mutie with a kitchen knife during a domestic dispute at their Kiambu apartment, where the family had lived for only about two weeks. He stabbed her in the heart.

Mary tried to run down the staircase but collapsed and died at the scene. Bloodstains on the stairs showed she had tried to escape.

Dr. Masoba was arrested and taken to court in Kiambu on August 10, 2026. Investigators asked for more time to gather evidence. He will remain in custody for seven days and must also undergo a psychiatric evaluation.

Mary left behind a three-year-old son….

I am biased here because I always use a *large* loading dose of vancomycin (when I use vanco instead of linezolid or daptomycin!) but I have not been able to use continuous vancomycin infusion (rare practice in US hospitals but fairly common in other countries)
