Psychiatry did not first discover psychological distress to be a set of medical disorders and then claim authority over it; it claimed authority over it and only then sought to justify it through medicine.

The initial attempt at this, by grounding psychological distress in demonstrable brain pathology, failed: nineteenth- and early twentieth-century psychiatry found no consistent lesions or disease processes.

Rather than relinquishing its jurisdiction, psychiatry started to use the term "mental illness"—a much looser and more ambiguous concept that appropriated psychosocial explanations into medicine while preserving psychiatry's professional authority.

Psychosocial forms of suffering were not shown to be medical conditions but were instead simply re-described using medical language, thereby maintaining medicine's claim to jurisdiction over them.

As people questioned medicine's role in psychosocial issues, from the 1980s on—after DSM-III and in the "Decade of the Brain"—a fresh attempt was made to ground psychological distress in demonstrable brain pathology.

Claims that "mental disorders are brain diseases" confidently reappeared. Yet, despite decades of research and billions of dollars in investment, no brain pathologies were identified for any of the major psychiatric diagnoses.

As this failure has become increasingly difficult to deny, more recently, psychiatry has relied on pragmatic and pluralistic definitions of 'disorder' that no longer require demonstrable pathology, while continuing to treat its medical jurisdiction as self-evident—as semantic and lacking of substance as the shift to the term "mental illness."

As such, psychiatry's claim to medical jurisdiction over psychological distress has never been empirically established. It has been repeatedly assumed, repeatedly pursued, but never demonstrated. We are therefore under no obligation to accept that jurisdiction simply because it has long been asserted.
