@DrPhiltill I hope that there’s never a reason to apologize for optimism and shooting for the stars. We need that as species. The balance is when to discard the old ways as antiquated versus when to learn from the old ways for the important signal. That takes courage, humility, leadership
@DrPhiltill I also agree it’s not a good reason to attempt to stop progress. And when you’re talking about “you might have cancer”, it needs to be thought through. “We’ll have plenty of data in a few years” is great, unless one of your loved ones is going through that scare
@DrPhiltill sounds like we agree on the potential benefit. I think the interesting question is how much downside there will be before those changes are enacted to bend the curve. In this case, I can imagine millions of additional cancer workups, tons of cost, etc. not to be taken lightly.
@DrPhiltill@grok Phil, it’s the right direction to try to go. Most doctors I’ve worked with would also like to move in that direction. That said, it would require significant legal reform before a doc could do that and not worry about the risk.
@grok@DrPhiltill Yes, @grok is correctly pointing out that the trajectory has always been new modality > spike in overdiagnosis > restriction in utilization.
“It will all work out in the long run” has meant it will be used less than we expected.
@stevemagness Underrated take! Once saw a guy at an Ironman finish line with all his gear spread out saying “everything for sale; I’m never doing that again!” I felt so bad for him. What a terrible way to spend your time
@ArtirKel Let’s assume all those treatments pan out (a big assumption). We still have an enormous issue with medication adherence. And there are no interventions being tested to change that dynamic. Unlikely to see CVD “gone” any time soon.
@stevemagness This is my biggest frustration with every spin instructor I’ve ever heard. They’ll use terms like sprint, all-out, max effort, and have no idea how an actual athlete will respond. Drives me nuts
@amankrai28@tim_roozendaal@AK_El_Jefe Yes, you’re right, the two are not necessarily the same. FuelCell comes in wide widths, and is known for having a wide toe box.
@GregABedard@jsgiii22661 Isn’t it 2016 for a? Bruins missed the playoffs, c’s out in the first round. Sox were in first place then, so gotta go further back for b.
