The HIFI-2 study, now published in European Urology Oncology, marks a major milestone in focal therapy. As the largest prospective clinical study of salvage HIFU following radiotherapy, it includes 531 patients across 30+ centers, offering real-world evidence.
Dr. Guillaume Ploussard, first author of the publication states, "This large prospective multicenter study demonstrates that salvage HIFU can achieve clinically meaningful oncologic control while maintaining an acceptable safety profile in patients with localized recurrence after radiotherapy."
This study reinforces the growing role of non-invasive, organ-preserving approaches such as @FocalOneHIFU in shaping the standard of care for prostate cancer management.

@medtechmd Still worried about utilization overall, seems they have no problem selling the machines but guidelines really stifle demand. Hopefully the RCT publish and registries can change that

It’s like a urologist civil war and it’s worrying that I will get different answers from everybody confusing me if I was a patient
@jryckman3@DrSpratticus@radoncreview Unfortunately that is where the US is failing, most European experts on HIFU have required multidisciplinary teams to start a focal therapy program. I know major centers that have HIFU here (Duke, MD Anderson, etc.) usually have full teams that offer all treatments
@jryckman3@DrSpratticus@radoncreview I completely agree, there is not much if any long term studies on HIFU and there needs to be. I’m hoping that FARP will be the first of many RCTs that extend into long term to give a better understanding of HIFU
@jryckman3@DrSpratticus@radoncreview I’m not vowing for HIFU to be a first-line treatment before long term results are verified but I do believe that if a general consensus on procedure and post-op follow is universally determined there can be benefit especially if prostate checks are routinely made

@jryckman3@DrSpratticus I completely agree that a patient should see a multidisciplinary team that can confer all procedures, but I do notice that focal therapy gets grouped together in one term but different modalities have very different outcomes which is why HAS only accepted HIFU and not others
@DongNguyeb@jryckman3@DrSpratticus Interesting, so when it comes to machine function they both generally go the same route but efficacy is where they differentiate, but that’s where I get confused cause the HAS confirmed efficacy for HIFU and the HAS specifically was designed to look at efficacy and not function.
@jryckman3@DrSpratticus As an outsider looking in, the US doesn’t consider HIFU for cancer treatment but in France the HAS has approved HIFU as a 1st-line treatment for early and intermediate.
