🚨New paper:
Everything you actually need to know about Cerebral Amyloid Angiopathy (CAA) — in 10 clinical takeaways. 🧠💥
🙏 Grateful to @StrokeAHA_ASA for the invitation — and for the intellectual freedom to share my expert perspective.

The 2025 @WorldStrokeOrg scientific statement on cerebral amyloid angiopathy (CAA) - a "very prevalent" small vessel disease - is discussed by @a_charidimou at #WSC2025, including stratification of future bleeding risk in probable CAA cases and the role of inflammation

🚨 New meta-analysis
🩸🧠 In selected patients with AFib &
cerebral amyloid angiopathy / intracranial hemorrhage, 🫀LAAO may be a safe and feasible alternative to 💊anticoagulation.
Excellent work @TheodorosMavri2 and team @IntJStroke!

Excited to present our newly published study/meta-analysis on LAAO in patients with previous ICrH and/or CAA.
Many thanks to my mentor @a_charidimou. Without his guidance this endeavor would not be possible.
I also want to thank Dr Archontakis-Barakakis and Dr Chlorogiannis...

🔹 GRE / SWI (Gradient Echo / Susceptibility Weighted Imaging)
These sequences detect magnetic distortion from paramagnetic substances—like iron, calcium, or blood.
🩸 Blood breakdown = iron = black on imaging
🧠 Use for:
– Microbleeds
– Hemorrhagic conversion
– Cavernomas
– Calcifications
Know your anatomy: iron is naturally present in regions like the globus pallidus.
🧠 The image below shows microhemorrhages in the thalamus and basal ganglia.
Those small, scattered, circular dark lesions are microbleeds.
The iron deposits distort the local magnetic field, leading to a signal dropout on SWI.

⭕️🧠 CAA progression & neuropath staging (CAA=Cerebral Amyloid Angiopathy)
Here is my take👇 which will try to unpack 🧵 in the coming days.
⛱️ CAA is not a single disease and is not uniform
🌂CAA does NOT invariably progress to bleeding as part of it's natural history, but rather there are hemorrhagic/advanced and non-hemorrhagic phenotypes which progress and behave distinctly
🧊 We only see the tip of the CAA iceberg

⚡️🩸🧠 Practical framework for diagnosing Cerebral Amyloid Angiopathy-related inflammation (CAA-ri)
We frequently discuss challenging CAA-ri cases with colleagues around the world. This framework summarizes key diagnostic features across the CAA-ri spectrum in a clinically useful manner.

Here is a pearl tutorial on CAA from the master himself. Plenty of CME credits here. Go challenge yourself. See if you have understood the Boston criteria adequately.
