CD38-negative MCL is linked to higher rates of TP53 alterations, distinct mutational profiles (NOTCH, CD79B, CXCR4), and features overlapping with leukemic non-nodal MCL.

An elderly female presented with erythematous cutaneous lesions on the chest. A bone marrow biopsy was submitted for review. Which marker do you want to put under the scope first??

Indeed, this is a case of BPDCN, a good mimicker for acute leukemia and rarely T cell lymphoma (CD4+). Recognizing the blastoid morphology and history of cutaneous involvement are pivotal. The cells can have cytoplasm tadpoles or pearl-like vacuoles, but these are not specific.

AML with MECOM (EVI1) translocation. Commonly associated with pronounced megakaryocytic dysplasia and hyperplasia. Some patients can present with thrombocytosis. The prognosis is poor.
