Indeed, this is a case of BPDCN, a good mimicker for acute leukemia and rarely T cell lymphoma (CD4+). Recognizing the blastoid morphology and history of cutaneous involvement are pivotal. The cells can have cytoplasm tadpoles or pearl-like vacuoles, but these are not specific.
