Current status: Googling different versions of..."Medicaid care navigation; mental health facilities NJ; emergency Dual eligible support" to help a family member in need--and coming up empty--As a 'healthcare person' I've never felt so uninformed and unable to help.

I know there are a number of start ups (and incumbents) working on these challenges and I'm eager to see more. That said, in this moment, I am seeing the gaps in the system that happen when patients are not IDed and enrolled early for preventive and ongoing support.
