Mona ElKalioby, @monaelkalioby, @CairoUniv, presents a challenging case of pediatric plaque #psoriasis associated with LRBA deficiency. For systemic condition, the patient was treated with systemic corticosteroids (methylprednisolone), immunosuppressives (cyclosporine and azathioprine) and IVIG. For psoriasis, sequential therapies were tried with topical calcipotriol/betamethasone ointment which showed only modest response. Despite increasing cyclosporine to 75 mg/day and trying acitretin and rapamycin, the psoriasis remained treatment resistant. During this period, the patient experienced another infection, triggering erythrodermic psoriasis, which required hospitalization. Treatment of the general condition and management of infection improved psoriasis, after which the patient was prepared for bone marrow transplantation.
ElKalioby emphasizes the importance of thorough history-taking and assessment in pediatric psoriasis and advises considering systemic disorders when systemic symptoms are present.
