Meanwhile, far away in rural villages of India a tournament that mirrored the FIFA Cup will reach its finals in the ancient city of Prayagraj where two of the best girls teams from 2350 teams of girls trained by @naandi_india under the project @NanhiKali will wear similar jerseys of Spain 🇪🇸 and Argentina 🇦🇷 and play their finals of what is now famously known in India as the Toofaan Cup - a unique grassroots football tournament for girls (many who never played any game before) trained by women coaches day in and out for few years to make them the stars they are today.

Last consult of the day, Z is now 6.5 years old, she had always puzzled us. We have been treating her as a myasthenia. Recently we looked into her genetic tests again, and she may have a titinoapthy - a mutation that alters the spring protein in our muscles; ‘titin’. Titin is the largest protein in the human body.

Her dad asked ‘how old are your newborn patients with rare diseases now, the ones you have looked after?’

There must be several dozens. Mind races and remembers a few.

The baby with severed limbs from an amniotic band disruption sequence was one of the first to feature as a picture on my PPT, she must be 24 years old, same age as the one we operated for gastroschisis (guts spilling out of the abdomen) when I was a PG at Vanivilas hospital. Alfie from a Kent hospital who developed chylothorax post a surgical procedure (lymph flooded his chest, surgeon has inadvertently nicked a hair like lymph vessel) He needed pipes plumbed between his thorax and abdomen which were manually milked (he looked like a jug with two handles) must now be 20 years old. The baby airlifted by Kuwaiti military to our London hospital thinking he needed a liver doctor, but then turned out to have neonatal McCune Albright with a mutation that had set his thyroid to an overdrive (this led to his bones melting into his blood) Prof and I looked like Dr House with this one. It won me a prize and a poster presentation at Leipzig. He must be 16 now. Baby Nair who fooled us initially to be weak from being a premature baby but had pompe’s disease (she missed out on specific treatment for 3 months) has survived to 11 years on a life saving expensive enzyme replacement therapy. A doctor’s baby who had toxic levels of ammonia making her convulse, in the end had a benign but rare diagnosis (we reported this rare carbonic anhydrous deficiency in an international journal) will be a healthy 7 year old now. Many more such including the baby whose seizures started on day 3 as he was about to go home and would wreck his brain, he was aggressively managed. He had a very abnormal brain MRI which has normalised. He is not only seizure free but very much normal at 9 months of age now. We had relentlessly pursued a difficult diagnosis and have now arrived at a rare mitochondrial depletion syndrome. My latest evolving diagnosis baby is only 7 weeks old and will have a rare condition that has tempered for now (work in progress)
What a tapestry!
Time contracts and expands but kids age one week, one month and one year at a time.
