My day as an intern on trauma surgery: managing the trauma floor, codes in the ICU, and a nice thoracotomy in the ED. I sewed up the man's chest before he was brought to the morgue. Might be my favorite rotation of intern year.
(7/x) One often overlooked tool to determine the hemodynamic impact of rapid Afib - an arterial line.
If there are periods where the ECG shows rapid ventricular response but the arterial line does not show corresponding pulsatility, this means that the rapidity of the Afib is likely impacting hemodynamics.
In this case, there are only ~13 heart beats in this strip for 33 ECG beats - thus only ~ 1 in 3 cardiac cycles result in ejection.
Here, Afib is almost certainly either a contributor or perhaps even sole driver of hypotension.
Also notice the narrow pulse pressure, another indicator of low stroke volume.

5) Try to be as extroverted as you can (within reason) to really get to know the residents and attendings. Building a connection with someone can really help your chances.
6) Sleep as much as you can!
@nick_pettinelli 1) Be on time (obvious but can be harder than you think when you're in a new city).
2) Treat the rotation as a month-long interview, some programs will truly use the rotation as this in place of a formal interview.
3) Hoppenfield's for prepping for cases. This book is incredible.
