@schowardjd Lesions are numerous, uniform, light‑yellow follicular plugs on a highly sebaceous area. In addition, the chronic, asymptomatic course with minimal response to topical tretinoin is a clue to pityriasis folliculorum. Use 1% ivermectin cream OD if no response in 3mo switch to oral.

35yo man w/ muscle weakness, fatigue & ⬆️weight diagnosed w/ hypothyroid myopathy (Hoffman’s syndrome) due to Hashimoto's disease. LT4 improved weakness, but muscle pseudohypertrophy persisted.🤔I expected improved pseudohypertrophy as the excess GAGs are metabolized & cleared😮
