While low chloride can activate the renin angiotensin aldosterone system,would not high chloride from 3% saline also risk afferent arteriole vasoconstriction and acute kidney injury? Curious how you weigh the benefits versus risks in real world use
@ross_prager That makes sense in RAAS-driven diuretic resistance, but wouldn’t the risks be higher in patients where fluid overload is ADH-mediated (e.g., SIADH) or due to cardiorenal syndrome? Just wondering how you decide which patients truly benefit from this approach.
