End of life discussions are often treated like last call. When they should be equivalent to discharge planning; ongoing. Our pts deserve more than one conversation or a casual uncomfortable announcement poorly respected. The end is just as important as living and beginning.

Complex vulnerable pts require comprehensive care, multidisciplinary collaboration, and clinical competency. Spend the extra time to connect the pt with the nutritionist, call the specialty you just referred the pt to, and educate yourself. Pts deserve that type clinician.

Today my grandmother passed away unexpectedly. It’s painful, but I’m more thankful to the ER team who allowed a dignified death by honoring her POLST form. She now gets to spend her Valentine’s Day with Grandpa in heaven. #herchoice #notubes
