Speaking with 1 of our NP’s yesterday she similarly told me she spends as much time in each visit as the Patient/POA need; this because it could be their last PCP visit. For some it’s a calling and redefines patient (resident) experience @PineParkHealth

Interviewed a dietary director for an assisted living community last week and he said, ‘I put love, care and time into each meal, each plate, because it’s important. For many of our residents my cooking will be the last meal they ever get to enjoy.’

Got chills right there in the interview. Can’t stop thinking about it. Excited to have him joining the team.

- transitions of care support (what happens if I need acutely head to hospital or SNF)
