It was a few years ago, I had a direct line report whose performance suddenly went south drastically and it was affecting the whole team.
He failed to send in his report on time and took forever to give feedback on a task.

Within a week his performance was back on track and after a few months he came back to thank me for giving him a chance to speak up about his challenges and struggles.
