"Medicine can have extraordinary meaning. But it cannot substitute for being present in your own life. The world may need us as physicians.
But the people who love us need us as ourselves.
And that is the role no one else can fill."
Brilliant - and much love to you

Iโve been a cop for 15 years. I pulled a guy over last night. He was doing 85 in a 55. Weaving. I walked up to the window ready to write a reckless driving ticket. Maybe even take him in. When he rolled down the window, he wasn't drunk. He was shaking. "My daughter," he gasped. "She's at Children's Hospital. The chemo isn't working. They called... they said I need to hurry." I looked at his eyes. You canโt fake that kind of terror. I folded my ticket book. "Follow me," I said. I got back in my cruiser, flipped on the lights and sirens, and I escorted him the 20 miles to the hospital. I cleared every intersection for him. We made a 30-minute trip in 15. He ran inside without looking back. I waited in the parking lot for an hour. Just in case. He came out later, saw me, and walked over. He looked hollowed out. "Did I make it?" I asked. "Yeah," he whispered. "I got to hold her hand while she went. You got me there." He tried to shake my hand, but he collapsed into my arms instead. Sometimes, to serve and protect means breaking the speed limit.
Iโve been a delivery driver for Pizza Hut for six years. You see a lot of strange things in this job, but you also see a lot of lonely people. Thereโs one regular, Mrs. Gable. Sheโs 84. She orders a small pepperoni pizza every single night at 5:30 PM. Last Wednesday, I pulled up to her driveway. The porch light wasn't on. That was weird. Mrs. Gable always leaves the light on. I knocked. No answer. I called her phone. No answer. Most drivers would have just left the pizza and walked away. Weโre on a timer, after all. But my gut told me something was wrong. I tried the door. It was unlocked. 'Mrs. Gable?' I shouted, stepping into the hallway. I found her on the living room floor. She had fallen hours ago and couldn't get to her phone. She was weak, dehydrated, and terrified. I called 911 and held her hand until the paramedics arrived. 'I was just so hungry,' she whispered to me as they loaded her onto the stretcher. I visited her in the hospital yesterday. She looked small in that big bed. 'I didn't order a pizza today,' she said, looking worried. 'It's on the house,' I told her, placing a small box on her tray table. Iโm not a doctor. Iโm just a guy who delivers pizza. But sometimes, paying attention to the small routine thingsโlike a porch light being offโis the only superhero power you need.
Minimum wage in 2005 was ยฃ5.05 and a Freddo was 10p. Meaning that one hour of work got you 50 Freddos.
Fast forward to 2025, minimum wage is ยฃ12.21 and a Freddo is 35p. Now one hour of work will only get you 34 of them.
That's a 142% rise in pay but a 250% rise in frog shaped chocolate bars.
If wages had kept up with the price Freddos, the minimum wage would be ยฃ17.50 an hour.
The economyโs in shambles and yes Iโm measuring it in chocolate frogs. But this post is about more than that. Too many people are working full time and struggling to survive because absolutely everything is increasing faster than wages.
Something needs to change.
New paper, health value of each minute of different physical activity intensities against 6 health outcomes.

Pretty much the whole industry (incl. @Google Fit @Apple Fitness+) assume that 1 minute of vigorous activity = 2 minutes of moderate activity, in terms of heart health value. Our UK Biobank analyses show that they may be far off, e.g. 1 minute of vigorous = 9.4 minutes of moderate = 94 minutes of light intensity activities, in terms fo type 2 diabetes risk. 1 min of vigorous = 7.8 mins of moderate = 73 minutes of light, in terms of CVD risk.

Itโs been a tough few weeks. My 10yo daughter was diagnosed with a very rare, aggressive cancer called interdigitating dendritic cell sarcoma (IDCS). Iโm reaching out to identify clinicians/patients who have encountered pediatric IDCS, indeterminate dendritic cell histiocytosis or other (non-LCH) histiocytic sarcomas cases.
I'm trying to understand non-surgical chemo and targeted therapy options, new pathology markers to better diagnose subtypes/treatments, and any data on progression in pediatric patients. Please feel free to share โ Iโm trying to cast a wide net due to the rarity of this condition and how little is known.

I'm so impressed with @imperialcollege
Ranked Number 2 in the world, overtaking Oxford and Cambridge.
Their top graduates are coming out with useful degrees like Computer Science, Aeronautical and Mechanical Engineering.
And it's really notable how practical and commercial they are - compared with theory-heavy courses elsewhere.
It's a huge win for the UK - we're now home to 3 of the global top 5 Universities.
