Obstetric cardiac arrest
Let’s look at the big picture….
So among 355,000 obstetric anaesthetic interventions in a year in UK (delivered to approximately 50% of all births & at all times of day and night almost exclusively in the NHS)
Incidence of obstetric cardiac arrest
-Epidural for labour
1 in 56,500
- any anaesthesia for LSCS
1 in 8,500
-All epidurals & spinals
1 in 17,000
-all GAs
1 in 1,200
These fall into ‘big data’
They are no substitute for an RCT in assessing safety (except no such RCT exists or is likely to)
There will clearly be confounding by intention (the very highest risk cases more likely to have GA)
…..but they do provide welcome evidence of the absolute safety of obstetric epidurals& spinals (& possibly relative safety compared to GA…)

This. 1000x over. When collating my 14 years worth of ARCP outcomes required to CCT I had to include one I had complained about. Despite an apology and an assurance the incorrect personal and derogatory comment would be removed. Guess which version was held on file…..
@RenukaDias @NHS_HealthEdEng For every single arcp I’ve had so far there has been an error in the comments eg ‘you’ve not don’t this’ when I have and anecdotally this is very common. Were I there I could point out the error - this is PARTICULARLY pertinent for LTFT trainees where requirements are pro rata
There’s a theme from me today at #Anaesthesia2022. Regional means you can avoid GA&opiates that contribute to post-op delirium. If you have to give GA you can reduce “dose” ++ which seems to improve post-op cognitive dysfunction. Blocks rock! @RegionalAnaesUK - Plan A blocks.
