Preoperative biliary drainage in perihilar cholangiocarcinoma: retrospective multicentre analysis

This retrospective multicentre cohort study evaluated the impact of preoperative biliary drainage on postoperative outcomes in patients undergoing resection with curative intent for perihilar cholangiocarcinoma. Leveraging data from 27 international high-volume hepatobiliary centres, and using both propensity score weighting and multivariable regression, it was demonstrated that preoperative biliary drainage is associated with a significantly higher risk of posthepatectomy liver failure. These findings question the routine use of biliary drainage and support further prospective evaluation of drainage-free surgical strategies in selected patients.
