Can implant design or our technique mitigate the problem upstream?
Can we allow controlled fracture collapse without lateralizing the lag screw too much. Will using a a blade, a single screw, two screws help?
Also how important is the proximal profile of the nail itself? Do you routinely bury the nail below the greater trochanter, and how much does proximal nail prominence contribute to persistent lateral hip pain?
And when someone does develop symptoms, what is your threshold for injection, screw exchange, partial removal, or complete explantation?
We spend a lot of time thinking about getting hip fracture implants in.
Maybe we should also be designing and implanting them with the possibility that someone may someday want them out.
