Maybe the best treatment for symptomatic hardware is preventing it in the first place.

This EFORT review raises an important issue after cephalomedullary fixation: once the fracture is healed, removing the implant may create a new biomechanical problem.

But what causes the symptoms that make us consider removal?

For me, one of the biggest issues is lateral prominence of the screw as the fracture collapses and shortens. The fracture is doing what we often want it to do mechanically—but the patient may end up with a prominent screw irritating the IT band and lateral soft tissues.

So perhaps the better question is:

Can implant design or our technique mitigate the problem upstream?

Can we allow controlled fracture collapse without lateralizing the lag screw too much. Will using a a blade, a single screw, two screws help?

Also how important is the proximal profile of the nail itself? Do you routinely bury the nail below the greater trochanter, and how much does proximal nail prominence contribute to persistent lateral hip pain?

And when someone does develop symptoms, what is your threshold for injection, screw exchange, partial removal, or complete explantation?

We spend a lot of time thinking about getting hip fracture implants in.

Maybe we should also be designing and implanting them with the possibility that someone may someday want them out.
