The job of the next FDA commissioner is actually quite simple: DO what Makary SAID, such as

1) plausible mechanism pathway and apply it to not just ultrarare diseases

2) super-quick approvals when it is obvious that something is efficacious and safe

3) quick first-in-human studies and a general re-think of separated phase 1,2,3

4) help make biologics cheaper

5) help people take healthcare decisions into their own hands (cash pay, wearables)

6) pave the way for the preventative medicine revolution
