In SWOG S1602, 2 BCG strains worked similarly in preventing recurrence, & adding intradermal “priming” didn't improve results. @ASCO#GU26#BladderCancer
AEs were slightly higher with Tokyo-172: helpful information as many centers face BCG shortages. -@pablombarrios@DanaFarber
Data from the ph3 LITESPARK-011 are finally out and brilliantly presented by Dr.@motzermd.
Belzutifan + lenvatinib vs. cabozantinib in ICI-pretreated advanced RCC :
- Significantly improved PFS (HR 0.70, p=.00007; median 14.8 mo vs 10.7 mo, IA2)
- Achieved higher ORR (52.6% vs 39.6%, IA1)
- DOR: median 23.0 mo vs 12.3 mo
- OS: more follow-up ongoing.
A major advance introducing a new treatment option in post-ICI RCC.

Induction ipi + nivo → CRT as bladder preservation in MIBC.
Indi-Blade: 2-year bladder-intact EFS 76%, and 2-year OS 96%.
ctDNA negativity identifies a group with 89% 2-year BI-EFS.
Systemic control first. Local consolidation second. Serious strategy.

The waterfall plot of DV in #HER2low urothelial cancer is extremely similar to the first ever data we saw with T-DXd in HER2-low breast cancer in 2018. Bodes well for the future of HER2 targeting in urothelial cancer.
After bladder removal surgery, some pts develop detectable tumor DNA in the blood. #GU26
In IMvigor011, atezo improved survival in ctDNA+ pts; clearing ctDNA during treatment was associated with better outcomes.
Blood-based monitoring may help personalize care. -@pablombarrios
In the RETAIN trials, a response-adapted bladder preservation strategy showed promising results. #GU26
ctDNA helped predict systemic recurrence risk, but did not reliably detect local bladder relapse.
Future strategies will likely require multimodal monitoring. @pablombarrios
About 60–80% of #BladderCancer express HER2. @ASCO#GU26
In a phase 2 study, disitamab vedotin led to responses in more than half of previously treated pts, even in HER2-low disease.
Toxicities were manageable, w/ fatigue % neuropathy most common. - @pablombarrios@DanaFarber
Before #BladderCancer surgery, could immunotherapy + targeted therapy improve outcomes? @ASCO#GU26
In KEYNOTE-B15, EV + pembro improved CR rates & survival vs standard chemo.
Side effects were common, so weighing benefit and risk remains essential. - @pablombarrios@DanaFarber
Outstanding presentation by @OncoBellmunt@DanaFarberNews
Studying ctDNA in IMVigor011 teaches us a lot:
- ctDNA- is a positive prognostic factor
- timing + concentration if ctDNA+ matter
@OncoAlert#GU26
Thought-provoking discussion by @JLWrightMD on ctDNA in MIBC: ctDNA is a systemic biomarker, is it a reliable bladder monitor? In RETAIN-1/2, 52% had local recurrence yet <10% were ctDNA+, and all metastatic events were preceded by local recurrence ==> cystoscopy/imaging remain essential.

Major practice changing peri-op readout by @MattGalsky, KEYNOTE-B15/EV-304:
EV+pembro peri-op vs neoadj GC + surgery → trial met endpoints with significant EFS (HR 0.53, p<0.001) + OS (0.65, p =0.0029) + pCR improvement (23.4% sig difference)
