Belzutifan + lenvatinib vs. cabozantinib in ICI-pretreated advanced RCC :
- Significantly improved PFS (HR 0.70, p=.00007; median 14.8 mo vs 10.7 mo, IA2)
- Achieved higher ORR (52.6% vs 39.6%, IA1)
- DOR: median 23.0 mo vs 12.3 mo
- OS: more follow-up ongoing.

A major advance introducing a new treatment option in post-ICI RCC.

Thought-provoking discussion by @JLWrightMD on ctDNA in MIBC: ctDNA is a systemic biomarker, is it a reliable bladder monitor? In RETAIN-1/2, 52% had local recurrence yet <10% were ctDNA+, and all metastatic events were preceded by local recurrence ==> cystoscopy/imaging remain essential.

ctDNA+ pts randomized to adjuvant atezo vs placebo had improved DFS (9.9 vs 4.8 mo, HR 0.64) and OS (32.8 vs 21.1 mo, HR 0.59).
