Presenting on variation on management of pancreatitis across the UK @asgbi today.
Cholecystectomy rates at 30days post gallstone pancreatitis are below 50% with a large amount of variation by trust.
Acute Pancreatitis 🇬🇧 a National Cohort Study of 30 days after presentation
🪨 Of 2.5k cases 50% stones, 22% idiopathic
🧯 Most are self limiting, recurrent attacks are a protective factor
😱 Mortality 2% predicted by age, male, frailty

-High mortality in severe group. Where are new therapeutics? (RAPID-I)
-Can we accurately work out who is going to have a severe course? (Personalisation/stratification)
All relevant to @LindAlliance psp

Interesting paper. Well done @misshollyman et al
Some takeaways for me:
-20% idiopathic; need strategies to make sure definitely isn't stones
-absolute no's high for lower severity disease. Big wins from changing care for this group Vs severe
But...

Panc Collaborative📜(113🏥, 2580😷) on acute pancreatitis shows 50% related to gall🪨, 20% idiopathic and 5% severe. ✝️2.3% overall, but >30% in severe cases! Age, ♂️& frailty predicted ⚰️:

We are excited to see the top ten research priorities in pancreatitis released.
The top three
-Pain relief
-Stopping disease progression
-Reducing inflammation
Lots of food for thought on the future direction of pancreatitis research.

Our dream comes true: the #WATERFALLtrial has been published in @NEJM Aggressive fluid resuscitation in acute pancreatitis is not safe & does not improve outcomes #GItwitter#SomosAEG@aegastro
What's behind this publication?
It all started 21 years ago

The NRC 2022 census of trainee / student-led collaboratives is now open (closes 17th July). For groups already affiliated, it should be in your inbox!
For new collaboratives that have sprung up, please complete the following online survey to get involved https://t.co/BFxriC6hWz
Key news from @DeMadaria and #WATERFALLtrial in acute pancreatitis. Stopped after interim analysis shows aggressive fluid arm (lactated Ringer's 20-mL/kg bolus over 2h followed by 3 mL/kg/hr) have 🔼necrosis, 🔼ICU admission and 🔼organ failure.
Bottom line: Use fluids wisely!
One of my takeaways from #ASGBI2022 is the value of collaborative research. Great to see the volume of data accrued in some big studies. Having been a local lead at @SalfordCO_NHS I’m biased towards @Panc_Study, thanks @misshollyman for presenting preliminary results. @asgbi
Angeliki Kosti presenting some of the initial @Panc_Study data at @asgbi in the @BJSurgery prize session.
Previous acute pancreatitis is protective against mortality if the patient has a subsequent attack.
