Today I learned :
Loop diuretics also inhibit NKCC2 channel in the macula densa thus activating RAAS. This occurs independent of the volume status. Thus for a loop diuretic to cause hypokalemia it doesn't need to cause volume depletion first.

Early steroid is key for ATIN. If in 1 week your patient doesn't respond as expected, start steroids. Early start, early taper.
