If you injected a person with Midazolam and morphine - for whatever reason - and they died, the death would be treated as suspicious and you would, quite possibly, be arrested for murder.

If a doctor or nurse injects a person with Midazolam and morphine (which, by the way, should NEVER be used concomitantly: check BNF guidelines), and the person dies, there’s no investigation whatsoever, even when bereaved families request one.
