The 3axis report has far more implications than I thought, and with the Supreme Court ruling, there are going to be legal challenges that take ages for any FTC action to be implemented.
All regulatory laws will be challenged.
They are running out the clock.
The fact that they have moved so much margin and spread to mail order and "specialty" shows that they are ready for PBM legislation and even if we get reimbursement reform it will be held up and challenged at every step.
Their ability to move margin is giving them the ability to price out small insurers, small PBMs, and pharmacies while bamboozling employers with tons of low cost maintenance meds and gouging on the prescriptions they require to be in-housed.
It's why we see the mail order facilities call for transfers or contact physicians directly to steal the prescriptions.
This pricing out is putting pressure across the entire landscape and positioning them to be the only entities left standing.
This is why they are gobbling up physicians offices. They will move margin to that side if the mail order practices become scrutinized.
This presents numerous problems. After physicians, they will go after the only thing left, hospitals. The poweful hospital lobby will find themselves defending revenue and practices of the very entities that are trying to consume them.
That's when they get behind a push for a single payer system. One that they run.
The tipping point is here, which is why they've applied so much pressure the last few years.

PBMs were legislated a kickback exemption that gave them a huge market advantage that was never meant to be given to pharmacies and insurers. Of course, there is no way for others to have significant leverage to negotiate while they possess that massive fulcrum.
The misaligned incentives and anticompetitive behavior is nothing less than racketeering and if something isn't done immediately, irreparable damage will be done to the healthcare infrastructure of this country.
I call upon everyone in healthcare, tech, and benefits to come up with a market based solution for employers. Communicate with one another. Share ideas. Collaborate.
We can not count on the government to save us. There is too much regulatory capture. We must all work together for a solution and disrupt the vampires.
Otherwise, we're all going to be under the thumb of CigUniVS.

Hate to say this, but the Medicare Advantage program seniors pick may be putting your local Indy pharmacy out of business. Next renewal, ask how much they pay pharmacies. And pharmacists need to help their senior patients figure this out
I would hand out flyers saying which MA plans support you the most.
Even worse, your employer has no idea that their insurance decisions are also putting pharmacies in bankruptcy
Not a single CEO I have spoken had any idea they were contributing to the death of Indy pharmacies
Indy pharmacies need to be proactive and educate local employers
It is wrong on so many levels that this is happening.
Time to fight back
