Lessons -
1. Pain referral patterns ie history / subjective (see my cheat table) - are super important - in power athlete women lower abdominal / pelvic pain strongly suggestive of pubic related driven groin pain, either joint or derived from pre-pubic tissues (in men add in inguinal disruption - old Gilmore's)
2. In women, when pain is provoked by both adductor squeeze AND crunch (abdominal muscles) then really it can only be pubic driven - these tissues attach to the irritable pubis or pre-pubic tissue complex
3. Contrary to popular belief, pubic related groin pain can be pain free on palpation. Negative for tenderness doesn't exclude it.
3. Ultrasound - in the right hands & patient with a favourable body habitus😄 - can be illuminating & can trump MRI.
4. US guided LA blocks can be incredibly useful & steer treatment as well as avoid unnecessary invasive treatment & going down non MSK rabbit holes.
