🔥 #EuroPCR 2026 Major Late-Breaking Trials: Long-term mortality following PCI with DES versus CABG for left main disease

An individual participant data meta-analysis of the SYNTAX, PRECOMBAT, NOBLE and EXCEL trials presented by Brian Bergmark showed no significant difference in all-cause mortality between DES PCI and CABG over 10-year follow-up in patients with left main coronary artery disease suitable for revascularisation.

In this new Journal Club article, @RuxSava and @Sticchi_Alex review this analysis which compared major adverse cardiac events in patients with significant coronary stenosis (FFR ≤ 0.80 or visual stenosis ≥ 90 %) versus those with non-significant stenosis (FFR > 0.80).

PROMISE trial: The first RCT in MINOCA showing improvement patients undergoing comprehensive assessment with stratified treatment. #TCT2025

In 80% there was underlying cause found:
- 35.6% Epicardial vasospasm
- 22.2% Unstable plaque
- 13.3% Spontaneous Coronary Artery Dissection

Comprehensive work-up let to change classification in 75.5% of patients, it consisted of:
- CAG and ventriculography
- OCT (in all cases) at the time of CAG
- intracoronary ACh provocative test (performed in all patients, unless clinically contraindicated
- TOE +/- contrast-enhanced echocardiography (CEE; if embolisation suspected: AF, mechanical valves, thrombophilic disorders
- blood sampling for circulating biomarkers and microRNA at the time of CAG or within 12 hours
- TTE in all patients
- CMR in all cases from day 3 to day 7
