Respected PM @narendramodi ,
Respected Health Minister @mansukhmandviya ,
May I suggest a doable solution to reduce the menace of antimicrobial resistance.
Due to rampant overuse and misuse of even the highest antimicrobials, bacteria and fungi are now showing extreme drug resistance leaving us with virtually no viable options.
But there is a way to make bacteria respond to existing antibiotics again.
It has been seen that if some Antimicrobials are not used for a prolonged period, organisms lose resistance and once again start responding to it.
Hospitals have been trying to rotate antibiotics being used in-house especially in ICU setup, but as long as they are still in use elsewhere in the community, the exposure to the drug continues and resistance continues to develop and persist. Moreover, transfer of resistant organisms to that establishment from environment does not stop and hence, doing this at individual hospital level proves ineffective.
We have a National Antibiotic Stewardship Program but as long as antibiotics remain available, doctors will continue to use them as per their past experience.
NHS UK is also taking drastic steps to curtail critical antibiotic usage by as much as 81% over the next five years. The WHO Access, Watch, Reserve (AWaRe) system groups antibiotics used in humans into three simple traffic light categories: Access (green), Watch (orange) or Reserve (red).
I suggest a mandatory National Antimicrobial Rotation Policy under which selected antibiotics and antifungals will be placed in highly restricted (Red) category for at least a full calendar year, during which they cannot be issued from pharmacy without submission of a documented positive culture report showing sensitivity to that antibiotic without any other available options.
The antibiotics in “red” list can change every year, coming into “orange” or “green” based on fresh community surveillance reports, while others go into the “red” list.
This should be across the board, including the antibiotics being used in veterinary, animal husbandry, rearing/slaughter, fisheries, and poultry industries. Disappearance of the antibiotic from environment altogether is needed for restoring sensitivity.
Such measures can be successful only by intervention at the highest level. I therefore appeal to you to include National Antimicrobial Rotation policy at the earliest as part of National Action Plan for Antimicrobial Resistance.
Regards, Dr Amit Thadhani.

Few years back i got a call from leading newspaper saying that we will publish your article on world heart day but you need to pay 75000 rs for that article , i refused - why i am writing in twitter because everyone should know that if you see a article by a doctor in a newspaper then its not necessary that he is a good doctor ,many times its a paid article.( i dont consider myself as a good doctor ) .
