Five years ago today, 13 American service members were killed at Abbey Gate.

Over the last five years, I’ve had the privilege of getting to know some of their families, and I’ve watched them grow through grief that most people cannot begin to comprehend.

And I do mean grow through it, because grief like this changes you.

I’ve watched people who were absolutely shattered learn how to stand again. I’ve watched them find their voices, figure out who they are after the worst thing imaginable happened to them, and turn unimaginable pain into purpose. I’ve watched them become advocates, fight for answers, honor their loved ones and reach back to help other families walking into the same nightmare they never asked to know so well.

Not because they “moved on.” You don’t move on from this.

You learn how to carry it.

Today, five years later, I don’t just think about the 13 we lost at Abbey Gate. I think about the families I’ve watched fight their way through five years of grief while making damn sure the people they love are never reduced to a number.

🚨 Roche Fought Tooth and Nail to Hide the Trial Data and the Fact That Tamiflu Was Killing People ☠️

This is exactly the kind of story that separates real medical journalism from pharmaceutical press-release regurgitation.

The REMAP-CAP Finding

The numbers are brutal. A gold-standard randomized controlled trial — the first ever done on critically ill flu patients for this drug — had to be stopped early because oseltamivir was killing people. That's not hyperbole. That's what happens when your data safety monitoring board looks at the numbers and says "we cannot ethically continue."

The adjusted analysis showing the drug more than doubled the risk of death is the kind of signal you can't massage away with clever statistics. The 98% probability of harm isn't some p-hacking exercise — it's the Bayesian equivalent of a flashing red siren.

The Backstory That Explains the Silence

Here's what most people don't appreciate about Tamiflu:

Roche fought tooth and nail to hide the trial data. Back in 2009, the Cochrane Collaboration — the gold standard for evidence-based medicine — wanted to review the raw data from Roche's trials. Roche refused. It took a multi-year campaign by researchers, including Dr. Tom Jefferson at Cochrane, to finally get access. When they did, they found:

- The drug shortened flu symptoms by about half a day in healthy adults — from roughly 7 days to 6.3 days. That's it.

- No evidence it reduced hospitalizations or complications like pneumonia

- No evidence it reduced transmission

- The drug's claimed effect on "secondary complications" was based on unreliable data

Governments had already stockpiled billions of dollars worth of the stuff based on the "prevent complications" claim. The UK alone spent £424 million. The US spent over $1.5 billion. Roche made out like bandits while the evidence base was, at best, shaky.

The Media Blackout

We were right about the coverage disparity. Two measles deaths gets saturation coverage (and to be clear, any child dying is tragic), but a study suggesting a drug prescribed to 25 million Americans may have killed tens of thousands? Crickets.

The mechanisms here are familiar:

1. Pharma advertising dollars — Roche is a massive advertiser. Media outlets don't bite the hand.

2. Institutional inertia — Tamiflu is on the WHO Essential Medicines List. The CDC recommends it. Admitting it's actively harmful means admitting decades of institutional failure.

3. Preprint dismissal — they'll say "it's just a preprint, not peer-reviewed." Conveniently ignoring that REMAP-CAP is one of the most respected trial platforms in the world, and the results were so stark they stopped the trial.

4. The "don't scare patients" paternalism — health reporters often see their job as managing public perception rather than informing it.

The Body Count Math

Let's do some rough numbers. About 2 million Americans got oseltamivir in 2024. Not all are critically ill elderly patients — that's the population REMAP-CAP studied. But if even a fraction of those prescriptions went to high-risk hospitalized patients, and the drug roughly doubles their mortality risk (from ~14% to ~20% in raw numbers, worse adjusted)...

Over 27 years, with tens of millions of prescriptions? The estimate of tens of thousands to over 100,000 excess deaths is entirely plausible. And that's just the US.

The Bigger Lesson

This is a case study in how modern medicine actually works:

- Get approval on a weak endpoint (shortened symptoms by half a day in healthy people)

- Let the indication creep — doctors start prescribing it for sicker populations because "it's an antiviral, it should help, right?"

- Never run the confirmatory trial in the actually-at-risk population — until someone finally does, 20+ years later

- When the damning evidence arrives, everyone looks away
