3 contributors to reflux that almost no one with reflux is told about.
1) Pressure
The lower esophageal sphincter (LES) is the valve that keeps things in the stomach from moving upward. It operates against pressure. When pressure increases excessively, it can overwhelm the valve and force acid and food into the esophagus.
The most common driver of this pressure buildup is gas from fermentation, which happens often with overgrowths in the small intestine.
Microbes fermenting carbs produce gas. That gas has nowhere to go but up.
2) Low stomach acid
This one is counterintuitive because reflux is almost universally assumed to be a problem of too much acid. But the LES closing mechanism is pH-dependent. The valve requires adequate acidity in the stomach to maintain its tone. When acidity decreases, the LES relaxes, allowing acid to reflux upward (even if it’s low).
Low stomach acid can also contribute to the pressure problem above.
Adequate acid is a primary defense against microbial overgrowth. When acid is low, microbes that should be killed on entry survive and increase in the intestine, increasing fermentation and gas production.
H. pylori infection, zinc depletion, chronic stress, and long-term PPI use commonly deplete acid.
3) Nervous system
The LES is not purely a mechanical valve. It is under direct nervous system control. The vagus nerve maintains LES tone during digestion. Sympathetic dominance (chronic fight-or-flight state) can suppress vagus nerve activity and can directly reduce LES pressure.
Vagus nerve dysfunction can also impact motility and delay stomach emptying. When food sits in the stomach longer than it should, pressure builds and reflux risk increases.
These are three common mechanisms. And they can exist simultaneously and compound each other.
The almost universal response to this is a proton pump inhibitor. PPIs will reduce acid symptoms in the short term by suppressing acid production almost entirely. But they address none of these mechanisms and can directly worsen several of them.
Lowering stomach acidity further reduces LES pressure, since it relies on adequate acidity to stay closed. It removes the primary antimicrobial defense of the upper gut, allowing overgrowth to expand unchecked. And it disrupts the downstream enzyme cascade, since adequate acid is required to trigger cholecystokinin release and the subsequent secretion of bile and pancreatic enzymes.
Long-term PPI use commonly contributes to dysbiosis, increased risk of C. difficile and other infections, depletion of nutrients like magnesium, B12, iron, calcium, and zinc and increased gut permeability.
Layering additional downstream problems on top of the original issues.
The irony is that for a lot of people, PPIs worsen the actual mechanisms driving their reflux while making the immediate symptom more manageable. Which is why so many people become dependent on them indefinitely with no end in sight.
Every dental issue falls into one of these:

It fires during fasting and sweeps bacteria out of your small intestine.
Hypothyroidism, diabetes, and vagal nerve dysfunction DESTROY MMC function.
And here's the vicious cycle nobody talks about...
Methanogenic bacteria like Methanobrevibacter smithii produce methane that further slows motility.
Slow motility feeds SIBO.

ROOT CAUSE 3: Poor bile flow.
Bile is your gut's natural antimicrobial detergent.
Produced by the liver, stored in the gallbladder, released into the small intestine.
It doesn't just digest fats.
It kills bacteria.
When bile flow is impaired — from liver dysfunction, gallbladder issues, or deficiencies in taurine, glycine, and vitamin C — bacteria accumulate unchecked in the small intestine.
Bitter foods, adequate protein, and liver support are not optional.
They are ESSENTIAL.
ROOT CAUSE 4: Immune dysfunction.
Secretory IgA (sIgA) is the antibody that coats your gut lining and neutralizes pathogens before they can colonize.
Low sIgA = open door for SIBO.
And what destroys sIgA?

ROOT CAUSE 6: Environmental toxins.
Antibiotics — even a single course — can disrupt your gut microbiome for YEARS.
Glyphosate acts as a broad-spectrum antibiotic in your gut, obliterating beneficial bacteria by inhibiting the EPSP synthase enzyme in the shikimic acid pathway.
That pathway is responsible for producing aromatic amino acids and vitamins.
Disrupt it and you disrupt neurotransmitters, antioxidant enzymes, and gut barrier integrity simultaneously.
Heavy metals, mold, and PPIs compound this destruction.
ROOT CAUSE 7: Coexisting conditions.
