So Bryan is basically an NPC. The whole movie is showing how a Resident Evil monster actually gets created, most people were expecting Leon or Claire. He’s just courier whose body becomes the next BOW.
The girl in the RV infects him early and the virus is cooking the whole night while he’s still running deliveries. By the time the scientists finish the cure, his body is already mutating.

He’s still conscious for a minute and begging them to stick him, but the tentacles take over before anyone can inject him

The voice note in the garage is the game collectible. That’s the tape/file a real protagonist would find later: some nobody recording a last message to his girlfriend before he turns. The movie is the story behind that item.

Cregger even said he didn’t want the ending where Bryan gets the needle and lives, because that felt cheap. The point was that you’d been watching a monster origin story the whole time. The cure existing doesn’t mean Bryan was ever getting it.
