When a chronically protruding LM stent precludes selective coronary engagement, TAVR planning gets complicated.
Our approach: intentional percutaneous stentectomy followed by DCB-only angioplasty, leaving a completely metal-free ostium for future coronary access.
TAVR was performed successfully.

In patients undergoing PCI for coronary bifurcation lesions, short DAPT (≤6 months) was associated with higher risks of MACE, death, and stent thrombosis.
In bifurcation PCI, anatomy matters. So does DAPT duration.
Still remains a key unanswered question: how DAPT duration should interact with PCI optimization by IVUS/OCT, bifurcation complexity, and 1-stent vs 2-stent strategies?

After treating a complex subtotal & calcified RCA with Rota-shock, I moved to the next challenge: LCx CTO
This time I pushed myself to apply the 3D wiring principles and used the ADAPT technique to understand tip direction, depth, and true spatial alignment.
I’m still early in my journey with angiographic 3D wiring (LCx probably the most challenging for 3DW), and there is a lot I need to improve but every case is an opportunity to develop precision, mental 3D visualization, and structured wiring strategy.
Huge respect for the experts who mastered this before us.
