Already scores >= 1 on sPESI criteria with SBP < 100mmHg. Any evidence of RV dysfunction on CTPA? I'd get an echo and troponin levels. If RV dysfunction detected and troponin is elevated then close monitoring and if any deterioration occurs -> Thrombolysis

@DocShivSharma Apparently he's covering two executive roles so he gets paid accordingly. I guess another question to ask is - When we cover the job of 2+ doctors because rota couldn't cover gaps, why do we not also get paid for the extra workload accordingly? 🤔

@kimsnurse @Long_way_round_ It's sad indeed but not surprising given the unreasonable appt demand and current staffing levels. I still don't understand how the expense of building a single digital system across the NHS is not seen as a money saving investment considering the work hours that would be saved!
