I met @SwanseabayNHS Chair and CEO in Nov 2021. Was told this nurses recruitment was under investigation&shouldnt have happened. No change-another complete failure at the expense of vulnerable families. Thank you bethan for highlighting this again. #wales @Eluned_Morgan #shame

Serious incident reviews should take 6 weeks-its been 20+. To advise more delays makes a mockery of its purpose-identify failings & prevent recurrence
