245 uncategorised incidents in 6 months. That could mean more are catastrophic or severe. Where is the accountability and the leadership? Where is the sense of urgency to learn and prevent mistakes? #maternity#nhs#wales#babyloss
Worrying maternity stats quietly released by @swanseabaynhs over Xmas. Breakdown of open incidents shows 245 incidents have no severity level attached to them. 39 are moderate or above with 9 catastrophic/death. 245 uncategorised highlights they have no clue safety/learning #nhs

@WGHealthandCare must be investigated for their part (or lack of) in the maternity crisis across Wales. Bullying, coercion, negligence and cover ups are rife across health boards, but no overarching body has paid attention and investigated.
There. Temperature check sorted
When @WGHealthandCare announce Chair of ‘Maternity temperature check’ this week it will be under a cloud
The decision makers (@CNOWales + NHS Performance & Improvement) missed warning signs in @SwanseabayNHS and didn’t listen in 2022
They excluded themselves from being assessed
@wesstreeting@SwanseabayNHS Furious that all these NHS Wales bodies who did and said nothing for years are not included in the review.
Babies and mothers died because they didn’t stand up and do anything.
My 2019hospital notes tell a very different story to the reports produced by the solicitor’s so called experts. The obstetricians reports they obtained were poor, contradictory, and seemed designed to minimise delays rather than reflect the truth. Instead of fighting for justice👎
🚨 Senior NHS managers are to be regulated by the @The_HCPC with a barring list to prevent those guilty of misconduct from working again in the NHS. @wesstreeting says this "will slam the door in the face of unsuitable managers"
@ChannonRobert@SwanseabayNHS It would appear from the review that they did nod along. We understood.
Team needs to be dismantled and head of the committee should stand down in disgrace. #btaw they could have spoken up. They should be ashamed.

Oh lord just got off a call with @nottspolice about the malicious deletion of files at NUH and let’s be fair the police have done what they can. But we still don’t know ‘who done it..?’…
Here are some numbers
1026 files were deleted
These belonged to 460 families
“The big mistake I’ve made in my first year is approaching that issue...in what I'd describe as a conventional way of doing government policy."
Health secretary @WesStreeting admits he mishandled the early stages of the government’s maternity policy.

This sweet boy, along with many other precious babies, is why we need a review into maternity services across the country. Had my concerns been listened to, and my symptoms investigated, Oakley would be here today, about to celebrate his 3rd birthday …
