This systematic review and bayesian meta-analysis found that cefepime was associated with higher odds of all-cause mortality compared with other β-lactams Meta-analysis of published peer-reviewed RCTs (73 trials, 15 411 patients) showed that cefepime was associated with a 98.6% posterior probability of higher mortality compared with other β-lactams (OR, 1.17; 95% credible interval, 1.02-1.34). Piperacillin–tazobactam and Hypokalemia.
A patient on piperacillin-tazobactam can develop progressive hypokalemia because the drug acts as a non-reabsorbable anion.
The high sodium load from the penicillin formulation increases distal tubular fluid flow and electronegativity, which drives profound renal potassium wasting that resists standard repletion until the antibiotic is stopped.
The patient is experiencing piperacillin-tazobactam-induced renal potassium wasting, a phenomenon driven by the chemical and pharmacological properties of the antibiotic.
When this occurs, aggressive potassium replacement often fails to normalize serum levels because the kidneys continue to actively excrete the supplementary potassium.

Because hydrogen ions (H⁺) are excreted alongside potassium to neutralize the negative electrical charge in the urine, patients with this condition classically develop hypokalemic metabolic alkalosis.
**Clinical Management Steps.
1. Check Magnesium Levels:
Always check and aggressively correct the patient's serum magnesium first.
Concomitant hypomagnesemia opens renal outer medullary potassium (ROMK) channels, allowing unchecked potassium wasting and rendering potassium replacement completely ineffective.
2. Consider Drug Discontinuation:
If the hypokalemia is severe, progressive, or causing ECG changes, discontinue piperacillin-tazobactam.
The tubular dysfunction is completely reversible; once the drug is stopped, serum potassium levels typically normalize rapidly within 24 to 48 hours.
3. Switch Antibiotic Therapy:
Transition the patient to an alternative broad-spectrum agent with a lower risk of renal anion-wasting, such as a carbapenem or cefepime, depending on culture and sensitivity data.

In traditional descriptions of the disease, the myelin sheaths surrounding nerve-cell axons are the main target; peripheral nerve myelin, although structurally similar, is unaffected.
The disease typically manifests with discrete focal attacks that affect vision, sensation, strength, and balance, and over time the frequency of attacks decreases and progressive worsening with prominent neuronal injury supervenes (seen in figure). In a minority of patients, symptoms are progressive from onset. Diagnosis rests on clinical criteria and magnetic resonance imaging (MRI) findings, supplemented by clonal expansions of immunoglobulin in cerebrospinal fluid and slowing in central nerve conduction velocities as measured by evoked potentials.
