68yo lady with DCM presented with cardiogenic shock and incessant slow VT (135 bpm Panel A). CRT-D previously implanted with surgically implanted #LVlead.

First #endocardial ablation has failed and suggested an epicardial origin (Panel B). Chest-CT reconstruction (Panel C) helped us to choose the best route. Fifth intercostal space left thoracotomy (Panel D) gave us access to LV substrate that was succesfully cryoablated.
