But, providers have a right to bill their UCR ( or cash fees).. though its never approved for actual pmnts from the insurance. Some ins bill pt for the difference, some no.

Health field pmnts are based on fee schedules that the insurances create.. provider can bill 1000$ for office visit, but ins pays as per fee schedule for that privider and maybe zip code, we all rcv EOB.. and it shows all these..
