Cholinergic basal forebrain integrity and cognition in Parkinson's. Have you thought about it? Slater and colleagues knock it out of the park w/ this paper and review MRI scanning as well as teach us about this super important and relevant topic.
Key Points:
- The cortical cholinergic system breaks down in Parkinson's and is related to thinking dysfunction.
- Postmortem brains and also alive PET scans have shown us the changes.
- The authors are interested in whether MRI can be helpful.
- They focus on T1- and diffusion-weighted MRI.
- Nucleus Basalis of Meynert (Ch4) volume when reduced is a marker for cognitive decline.
My take: I love this paper, and particularly the 'money figure' showing 1- Ch4 and projections are intact w/ normal cognition, 2- In mild cognitive impairment, the timing of CH4 cell loss and projections are unclear when trying to correlate them to percolating dementia, 3- finally there is decreased Ch4 cell count and the loss of the integrity of the projections emerges when dementia is present. It will be interesting to observe as MRI technology evolves, will our ability to correlate cognitive symptoms improve. Shaking the magic eight ball, 'it is likely.'
