I can’t believe @wearfigs made matching scrunchies to go with their scrubs like I truly am the target demographic for this I have a matching pair of earrings for every set of scrubs too…

Spotify wrapped dropping the same day as fellowship Match is the worst because now I have to go through all of these lame stories to find out where my friends are going 😭
Lesson 3: Most problems that occur in the ICU are not related to ‘volume status’ despite the disproportionate amount of time on rounds we spend discussing them.
When I was a fellow, I recorded what percentage of time we spent on rounds discussing volume status: fluids, Lasix, free H20 etc. It was about 30%.
MOST patients in the ICU are not fluid responsive.
MOST patients in the ICU acquire volume overload.
At the end of the day, despite how much we want to dress it up, salt water does NOT cure sepsis. It does NOT attenuate multi-organ failure.
We need to spend less time agonizing over decisions to give (or withhold) slight amounts of salt water and focus on treating the underlying cause while MINIMIZING volume overload.
Thinking about how I took a covid course in med school and learned from Dr. Hotez that COVID’s R factor was like 1-2 while measles is 12+ so in terms of how contagious COVID is… it’s nothing too crazy but measles?? Thank god we have a vaccine. Anyway fast forward to 2025 oops
I missed this when it came out: 2024 HHS/DKA guidelines:
No more "the gap has closed".
Following the anion gap is no longer recommended.
Instead, follow plasma ketones directly, venous pH, and/or bicarbonate.
Zoledronate administered every 12 to 18 months prevents fractures in older women. Ten years after initiation of this trial, zoledronate administered at baseline and 5 years prevented vertebral fracture. Full trial results: https://t.co/53eWKHkVd3
None of our clinic patients came because of the weather today so I have spent the last 20 minutes reading and then excitedly telling my senior about this study lol I need to remember this moment when I decide on what fellowship to do
Taking step 3 very soon so I started the ccs cases and they’re low key so fun I don’t feel like I’m studying it’s something between playing a game and morning report—two things I greatly enjoy
This ended up in my evaluations as me failing to “proactively examine” my patient when asked for an assessment 😒 things like this really make you want to quit residency lol ridiculous
I asked my medical student to go ask a patient’s mom if the patient pooped. This student is following this patient. When my attending asked me the same question, I told him the student is asking right this moment. My attending told me I’m not allowed to do that ever. Students
@apsmunro For whatever reason the peds dept in my institution resorts to art sticks for random regular labs too like… a BMP was hemolyzed from the heel stick so the K looks crazy go do an art stick on this infant? Why can’t we ask nursing for a venous blood draw first?

The care I am proving to my patients. He constantly has to exert his dominance and make it an unsafe learning environment for us all and I hate it and want to put it in my ratings/feedback. But I’ll have to work with him again during the next 4 years, so I won’t because I worry
